Nurse Practitioner for Family Health Team

Join the Peterborough Family Health Team as a Nurse Practitioner dedicated to enhancing access to primary care. This 6-month contract offers a supportive team environment focused on patient wellness.

As a part-time position at 30 hours per week, you'll work closely with family physicians and a multi-disciplinary team. Your responsibilities will include health screenings, chronic disease management, and evidence-based nursing strategies. Play a crucial role in patient advocacy and care delivery.

Key Responsibilities:

• Perform health assessments with thorough history-taking

• Implement health promotion and prevention strategies

• Collaborate with physicians for complex cases

• Order and interpret diagnostic tests

• Regular attendance at Family Health Team meetings

Requirements:

• RN registration in the Extended Class with Ontario

• Successfully completed Nurse Practitioner education

• 3-5 years of primary care or rural nursing experience

• Willingness to work flexible hours as needed

• Supportive of vaccination initiatives against COVID-19
