Home Care Nurse Patient Visits

Support patients at home through scheduled community visits, relationship-based nursing, and individualized care planning as a Registered Practical Nurse. Full-time opportunities are available with flexible scheduling options.

Para Med’s community nursing work serves patients of all ages across various settings. As a Registered Practical Nurse, you will address patient needs within your scope and receive Registered Nurse support for more complex care. Your practice will help patients remain comfortable at home while developing clinical skills and therapeutic relationships.

• Build rapport and trust through collaborative patient engagement
• Deliver safe, ethical care during community visits
• Apply assessment tools to identify individual needs
• Execute, monitor, and adjust patient care plans
• Update records related to encounters and care plans

• CNO registration as a Registered Practical Nurse
• Experience with community care considered an asset
• Experience working with vulnerable populations
• Reliability and flexibility for scheduled community care
• Willingness to travel between patient residences
