As a Case Manager, you will be instrumental in supporting social assistance participants through comprehensive assessments and action plans. Collaborating with various service partners, you’ll provide wrap-around support while navigating complex social systems. Your focus will include housing stabilization, crisis management, and conflict resolution, helping individuals reduce barriers and enhance their stability.
Key Responsibilities:
• Conduct comprehensive assessments for social assistance participants
• Develop personalized action plans to achieve client goals
• Liaise with community agencies and service partners
• Provide wrap-around support and systems navigation
• Maintain thorough documentation and manage financial entitlements
Requirements:
• Minimum two-year diploma in Human Services
• At least two years of experience in social services
• Experience with Trauma-Informed Care and Strengths-Based approaches
• Eligibility for registration with Ontario College of Social Workers is an asset
• Strong skills in coaching and counseling in diverse contexts
