Advanced Nurse Practitioner

• You MUST have a past experience of working as a prescribing ANP in a GP surgery or OOH setting in UK.
• You will be responsible for attending to patients who wish to be seen bya health care professional; diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs.
• Prioritising and triaging the needs of patients accordingly, making any necessary referrals for investigations inthe appropriate manner.
• Working with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

• Provides patients with access toa Nurse Practitioner in surgery and over the phone for all presentations in primary care.
• Controls admission and discharge to own caseload through:
• Appropriate comprehensive assessment to include physical, mental, emotional andsocial health including the use of invasive and non-invasive investigations to establish a diagnosis.
• Prescription of evidence-based care including medication.
• Developing with the patient and family when appropriate, an ongoing plan for health with an emphasis on prevention and self-care.
• Follow-upand evaluation of care.
• Direct referral to and liaison with consultants and other professional agencies.
• Making and accepting referrals within the practice team.
• If qualified to, prescribes according to formulary and competencies.
• Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
• Support andmange health needs of women presenting for family planning, or sexual health consultation.
• Assess,identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
• Acollaborative, supportive and holistic approach.
• To keep accurate patient records on computer system.

The Practice has a list size of 11,500 patients. The Practice Team consists of 2 GP Partners, 5 Salaried GPs, a Nursing Team of ANPs, HCAs, Pharmacy Team and a well-established administration team. We inhabit modern premises which are located in Serpentine Green Centre, Hampton, Peterborough, where there is good access to shops, sporting facilites and restaurants. There is plenty of secure, free parking to the rear of the Practice.

Pathological specimens and investigatory procedures

• Undertakes the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs.
• Checks andprocesses results received via Systm One as instructed by the GPs.

Administrative Responsibilities

• Disseminates learning and information gained to other team members in order to share good practice and inform others about current and future developments/courses.
• Manages and assess risk within areas of responsibility,ensuring adequate measures are in place to protect staff and patients.
