We are looking for enthusiastic and committed practitioners to join our Family Hub Teams supporting children and Families to deliver services in community settings. The Outreach Worker will work with children, young people and families aged 0-19 (or 25 with special educational needs and disabilities), assessing their needs and delivering high-quality interventions predominately as a group basis, They will also help families navigate to appropriate services within and beyond the hub to ensure their needs are fully supported.

WHAT

IS INVOLVED?

You will

  • Plan, deliver and evaluate group-based programmes to children, young people, parents / carers and families, which will be in community venues and family homes.
  • To home visit families in our local communities to support families to access services and reduce barriers to accessibility, including work with dads and male carers.
  • Work in partnership with children, young people, their families and carers providing direct help and interventions to families that contribute to meaningful positive change.
  • Work in an integrated way with other professionals to meet the needs of children, young people and families, so that they experience seamless support, e.g. access to nursery provision and deliver weekend and evening activities.
  • Regularly review the effectiveness of delivery for families in Community Settings.

WHAT WILL I NEED?

You will have:

  • A Level 3 qualification in a relevant field (e.g. childcare, early years, health, youth work)
  • Experience working with children aged 0–5 and families
  • Experience of multi-agency working
  • Experience delivering group activities or interventions
  • Strong communication and interpersonal skills
