Family Transitions Inc is seeking a Program Manager / QDDP to oversee client services for individuals diagnosed with intellectual and developmental disabilities, mental health issues, and challenging behaviors.
Job Type: Part-Time

- Manage the day‑to‑day client chart in accordance with program policy and procedures.
- Coordinate a full range of rehabilitative and behavior management programs and health care services, including those for intellectual/developmental disabilities, mental health issues, and challenging behaviors.
- Interact with individuals in a professional manner, adhering to all human rights guidelines.
- Support the Program Supervisor/Executive Director in the development of assessments, annual forms, discharge planning, and individual service plans to ensure appropriate services.
- Implement and monitor person‑centered Individual Service Plans.
- Facilitate new employee orientation for Direct Support Professionals.
- Communicate effectively with all community and Family Transition, Inc. contacts.
- Complete all required clinical documentation and follow Health, Safety, and Emergency guidelines.
- Assist the Program Supervisor in day‑to‑day operations of the Day Support Program as needed.
- Assist the Program Director/Executive Director in creating clinical documentation such as Part V individual plans.
- Complete intake/admission paperwork at the time of admission into the program.
- Prepare client quarterly reports within 90 days after admission.
- Submit Serious Incident reports into CHRIS within 24 hours and complete internal investigations.
- Facilitate staff meetings and participate in bi‑weekly administrative meetings.
- Perform other duties as assigned.
Education and Experience: Minimum of five years’ experience in a mental health/intellectual disability field, combining administrative/management, staff supervision, evaluation, and direct service experience, including crisis intervention. Experience with public interactions required. Valid driver’s license with an acceptable record.
Knowledge, Skills, and Abilities- Effective oral and written communication.
- Demonstrated leadership in project management, program goal setting, and action planning.
- Ability to plan, manage, and evaluate programs, including assessment.
- Knowledge of developmental and intellectual disabilities, classifications, effects, and individual needs.
- Knowledge of employment principles and best practices for programs and service delivery.
- Knowledge of applicable federal, state, and local guidelines, laws, and regulations, including person‑centered practice, Medicaid waivers, and positive behavioral supports.
Family Transition, Inc. requires all employees and contractors to undergo a criminal background check and a Department of Social Services (DSS) Central Registry of child abuse and neglect complaints check. Certifications and Health Requirements- Completion of a Tuberculosis Screening Test upon employment and annually thereafter.
- Current certifications in behavior management, CPR, and first aid.
