Position Summary:

Under direct supervision, provides personal and homemaking services to ill, convalescing, elderly, and disabled persons in the patients' home.

Essential Functions and Responsibilities :

• Provides personal care of a patient, including bathing, grooming/hygiene, and toileting.
• Takes and records daily vital signs. Reports all vital signs that are outside the normal range.
• Assists patients with positioning/turning, transfers, catheter care, and range of motion exercises.

• High school diploma or equivalent.
• Successful completion of a written home health/hospice competency test and the clinical skills testing.
• Certification as Home Health Aide (CHHA).
• State of Michigan CNA Certification
• Current state driver’s license, proof of insurance, a reliable vehicle and be a responsible driver.

• Six months of experience with a variety of patients.
