The Assistant Director of Health and Wellbeing plans, organizes and coordinates the nursing process to provide excellent care to residents based on The Waters Way of service, presence and partnership. Through clinical and operational immersion, this role builds upon the RN role to enhance resident outcomes, ensure appropriate care, complete assessments, modify care plans and conduct internal audits for regulatory compliance.
The role is a key contact for resident family members and physicians and participates in daily problem solving with residents and team members.
The Assistant Director of Health and Wellbeing provides exceptional customer service with courtesy, dignity and respect.
Responsibilities include:
- Manage resident care with expertise in assessments, symptom management, crisis intervention and family intervention
- Directly manage community Resident Assistants (RA) and guide the interdisciplinary care team in implementing The Waters Way, including health and wellbeing workflows, supporting technology, integrative nursing principles, and team training
- Lead shift planning, model person-centered care, oversee daily team schedules, and evaluate bedside handoffs to improve transitions of care
- Assist with hiring, onboarding, retention and performance coaching of Resident Assistants in collaboration with the Director of Health and Wellbeing; deliver performance evaluations and disciplinary actions
- Participate with other healthcare providers in developing and modifying resident care plans
- Model healthy personal behaviors and coach the team to achieve Thrive outcomes for residents and staff
- Investigate and report suspected resident abuse or neglect per policies
- Audit nursing practices for regulatory compliance and update policies and training gaps with Director of Health and Wellbeing/Home Care Manager
- Identify safety gaps in administration, documentation, medication, physician orders, assessments, and care plans
- Maintain a customized caseload and clinical presence to support integrative nursing modalities and evidence-based practices
- Follow HIPAA policies to protect resident privacy
- Respond to emergencies outside of daily duties, including emergency calls, security issues, and falls
- Assist with fall logs and MAARC documentation under direction of leadership
- Attend required meetings, trainings, and conferences
- Available for occasional phone consultations when clinical needs arise
- Complete all training programs by assigned due dates
- Ability to take On-Call responsibilities per defined schedule
- Perform other duties as assigned
- Graduate of an accredited school of nursing required
- Licensed as a Registered Nurse (RN) in good standing by the state where the community is located
- Minimum of two years of general nursing experience; geriatric nursing experience preferred
- Supervisory and leadership experience preferred
- Knowledge of applicable state and federal regulations, including the Home Care Bill of Rights as it applies to the community's nursing practice
- Knowledge of and accuracy in administration of medications and performance of treatments
- Knowledge of and willingness to learn advanced therapies for memory care and integrative nursing therapies
- Excellent basic nursing care skills and advanced care ability within the scope of practice
