The goal of the program is to better serve the many communities throughout Wisconsin by helping to keep every free and charitable clinic as well as health department sustainable. WAFCC does this in many ways via helping with things such as: helping to foster and develop community and inter-clinic collaboration, helping to implement best practices in the form of our standards of excellence, helping with programmatic development via policies and procedures, implementing new programs, engaging with the community, and much more.
Member Duties : Clean, process and analyze EMR data that supports quality measurement reporting and improvement initiatives, ongoing maintenance of accreditation, regulatory requirements, and priority programs. Work with team to produce data visualizations and storytelling to aid in ongoing support and improvement. Serve as the clinic lead in helping capture and share patient success stories and RRCC team support to showcase patient resilience, clinic impact, and the importance of rural community health.
Work with RRCC Primary Care Providers (PCPs) and Community Health Partners to coordinate patient care orders and track completion of results. Take a specific, focused effort on prenatal visit follow-up and continuation of care post labor and delivery (for mother and baby). Using pulled listservs, identify patients overdue for cancer screenings and engage in targeted outreach to support patient completion of screening and capture of results for the primary care team.
