PRN Physical Therapist (PT) at Enhance Rehabilitation
Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a PRN Physical Therapist, you will provide high‑quality care for patients in a skilled nursing facility setting, evaluating and treating patients following individualized plans of care.
Responsibilities- Providing excellent care for patients
- Delivering outstanding customer service to patients and families
- Collaborating with facility staff to ensure excellent care
- Evaluating new patients, developing plans of care, and setting measurable, functional goals
- Treating patients, answering their concerns, and educating them on injury, progress, and probable outcomes
- Supervising Physical Therapist Assistants
- Re‑evaluating patient progress, revising plans of care, and resetting goals
- Completing timely and accurate documentation including evaluations, re‑evaluations, discharges, and daily progress notes
