Overview
St. Clare Health Mission is a non-profit organization dedicated to improving community health outcomes. This Ameri Corps position will focus on building capacity to address the growing epidemic of Type 2 diabetes within our service area, particularly among underserved populations. The member will play a pivotal role in developing and implementing a Diabetic Collaborative Practice Agreement (CPA) and leading the clinic-wide transition to a new Electronic Health Record (EHR) system.
The goal of this project is to create sustainable systems that will enhance patient care, streamline clinical workflows, and improve data collection for program evaluation and grant reporting. This is a capacity-building role, meaning the member will not provide direct patient care but will instead create and improve the systems and processes that enable the organization to better serve its community.
The Ameri Corps program supports service in alignment with the organization’s mission and goals throughout the service period.
Responsibilities- Lead development and implementation of a Collaborative Practice Agreement (CPA) for chronic disease management, with emphasis on diabetes care. Collaborate with physicians, nurses, and pharmacists to draft/finalize the CPA, design staff training, and create systems to monitor compliance and outcomes.
- Serve as project manager for the clinic’s transition to a new EHR system (main and mobile clinics), coordinating with vendors to ensure smooth rollout, developing workflows/templates, and providing training and implementation support.
- Partner with leadership to define key performance indicators (KPIs), oversee data collection and analysis in the EHR, and prepare program evaluation reports for grants and compliance.
