Forensic Coordinator - Waukesha and Milwaukee Counties

Wisconsin Community Services, Inc. (WCS) invites applicants to join our team as a Forensic Coordinator supporting multiple reentry programs across Milwaukee, Waukesha, Racine and Kenosha counties.

Job Summary

Provide ongoing service coordination, treatment planning, advocacy and monitoring to residents participating in the OARS, Conditional Release (CR), Outpatient Competency Restoration (OCRP) and Jail-Based Competency Restoration (JBCR) programs, ensuring evidence‑based care and compliance with Department of Health Services (DHS) contract requirements.

Essential Duties and Responsibilities
  • Coordinate, plan, and ensure follow‑through with community treatment for program participants.
  • Develop a supportive one‑to‑one therapeutic relationship with each participant, incorporating Motivational Interviewing (MI), Trauma‑Informed Care (TIC), and Person‑Centered Planning.
  • Assess ongoing treatment needs and compliance, providing assessment, treatment planning, assertive case management, symptom management, medication monitoring, crisis intervention, and coordination of multidisciplinary team meetings.
  • Develop and submit court documents for CR and competency participants, including predisposition investigations, treatment plans, adjustments, summary reports, discharge/transition plans, and status reports.
  • Coordinate team staffing with DHS/Department of Community Corrections (DCC) and other treatment team members.
  • Maintain safety practices and continually assess potential risk while working in the community.
  • Coordinate community support services, referrals, and assist with activities of daily living such as housing, money management, vocational/educational pursuits, scheduling, and transportation.
  • Encourage and assist participants in developing a natural support system.
  • Maintain participant service documents such as case notes, assessments, family home justification forms, release plans, ISP, QPN, crisis plans, budgets, risk assessments, and release of information forms within required time frames.
  • Ensure compliance with DHS quality standards, DHS Connect (EHR), SharePoint, and communication protocols.
  • Facilitate maximum participant financial contribution and third‑party payment by applying for insurance benefits and available assistance programs.
  • Provide timely communication with treatment providers, DOC/DCC, mental health institution staff, court personnel, and other agencies, responding to email and phone messages within 24 hours.
  • Conduct frequent home visits and transportation of participants using personal vehicles.
  • Develop MI skills through training, coaching, and participant contact, attaining basic fidelity within 12 months and advanced fidelity within 24 months per program contract.
  • Participate in MI coaching sessions, complete DHS surveys, audio recordings, and tests of knowledge as required.
  • Build positive relationships with stakeholders and funders, and complete necessary medical records and lab results per protocol.
  • Provide rotational crisis line coverage and testify at court hearings as needed.
  • Attend training, meetings, staffing sessions, and new staff shadowing rotations.
  • Other related duties as required for full effective performance.
Required Qualifications
  • Bachelor’s degree in social work, psychology, or a related human services field.
  • Experience in case management and service coordination with individuals experiencing mental illness, substance use disorder, or other special populations.
  • Valid driver’s license, automobile, and insurance sufficient to meet agency requirements.
  • Pass all employee requirements including references, criminal background check, and driver’s license check.

  • Communication: Effectively convey information to diverse populations and stakeholders, both written and oral.
  • Technological Aptitude: Use general technology for email, internet, and agency systems such as Microsoft 365.
