The LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well‑being of each resident as determined by resident assessments and individual plans of care.

• During rounds observes, assesses, documents, and reports resident condition/changes
• Assures implementation of care plans as directed
• Administers medications as prescribed
• Conducts rounds with physician and records visits in clinical records
• Performs narcotic reconciliation with incoming and outgoing shifts
• Identifies significant changes in the condition of residents and takes necessary action per facility policies and procedures
• Oversees and monitors aides
• Initiates and completes Incident and Accident Reports per facility policy
• Discharges residents per facility policy
• Completes Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
• Communicates with resident’s point of contact for status updates or changes in condition
• Other special projects or functions can be assigned at times by the Director of Nurses or the Administrator

Educational/Training Requirements:

• Completed an LVN program at an accredited vocational school or college

Licensing Requirements:

• Active Licensed Vocational Nurse in Texas in good standing

Experience Requirements:

• Experience working in a skilled nursing facility is preferred
