The In-Home Services RN delivers skilled nursing care in accordance with our clients standards and applicable regulations. This role involves direct participant care in homes and community settings, clinical assessments, coordination with interdisciplinary teams, and oversight of care delivery.

• EPIC experience preferred

• Must be comfortable in a home health setting.

• Pay: $48-51/hr rates are negotiable based on experience.
Responsibilities

• Provide direct skilled nursing care to participants in their homes and community.

• Assess, plan, and evaluate participant needs to ensure care is delivered in the least restrictive environment.

• Implement physician orders and administer prescribed medications and treatments.

• Educate participants and families on medications, treatment plans, and disease management.

• Coordinate outside appointments, hospital admissions, and specialist referrals.

• Monitor and document participant progress and report changes in condition to physicians and care teams.

• Supervise and support LPNs, HHAs, and CNAs per regulatory requirements.

• Participate in quality reviews, care conferences, and interdisciplinary collaboration.

• Maintain accurate and timely documentation to meet all charting requirements.

• Ensure consistent, high-quality care across the home-care team.
Requirements

• Associate’s Degree in Nursing (ADN) required; BSN preferred

• Active RN license in the PA or compact license

• Current First Aid and BLS certification (American Heart Association or American Red Cross)

• Valid driver’s license, reliable transportation, and proof of auto insurance

• Minimum 1 year of experience working with geriatric or frail populations

• 3+ years of healthcare experience with an emphasis in geriatrics preferred

• Bilingual ability (Spanish, Russian, Punjabi, or Hmong) a plus
EPIC experience preferred Must be comfortable in a home health setting. No OASIS experience required.
