Help older adults and families move through complicated care transitions
Care Connect helps families navigate senior living and care transitions. We are hiring for two part-time associate openings across our care navigation, operations, and sales recruiting tracks, with the goal of developing successful hires into long-term Care Coordinators.
This position is a strong fit for someone interested in social work, human services, healthcare administration, or operations who enjoys solving practical problems, speaking with people, and bringing order to complicated situations.
You will support the team with tour scheduling, documentation, referral updates, and facility research. As you develop the necessary skills, you will take on supervised Medicaid placement coordination, substantive phone follow-up, and more complex case assignments.
This role also includes outbound professional outreach, event participation, and basic marketing support. All associates contribute to both responsive client service and the growth of Care Connect.
Our target is to evaluate readiness for advancement into a Care Coordinator role around month nine. Advancement depends on demonstrated skills, performance, and business needs; it is not automatic or guaranteed.
