Clinical Pharmacist ! Pharmacy Phoenix Note Quick

Shift: Mon-Fri, Sat, Sun, 6:30am-11:30pm

We are looking for a dedicated inpatient clinical pharmacist to join our team at the Thomas Campus, supporting the medication needs of our pediatric patients. 8-hour shifts, evening differential, relocation assistance, starting bonus up to $10,000.

• Review and manage medication orders for therapeutic appropriateness using age, weight, height, lab values, diagnosis; intervene as necessary.
• Effectively screen for and manage clinical alerts (allergies, interactions, duplications); problem‑solve medication orders and recommend alternatives.
• Comply with Pharmacy and Therapeutics Committee directives related to formulary management.
• Document clinical interventions to meet cost containment and evidence‑based goals.
• Prepare and dispense sterile and non‑sterile medications in compliance with policies and laws.
• Provide education and medication counseling to patients, caregivers, and staff; support antimicrobial stewardship.
• Assist in developing guidelines, policies, and intervention strategies for medication use.
• Supervise technical, intern, and support staff; coordinate workflow.

• Bachelor’s Degree in Pharmacy (Pharm.
  
  D.) or graduating within 6 months.
• Postgraduate Year 1 (PGY‑1) pharmacy residency program accredited by ASHP (preferred).
• Experience with automated information systems.
• One year of hospital pharmacy practice or equivalent.
• One year of pediatric patient population experience or equivalent.
• Current licensure as a Pharmacist (Arizona State Board of Pharmacy).
• Basic Life Support (BLS) and Pediatric Advanced Life Support (PALS) or Advanced Cardiac Life Support (ACLS) (preferred).
• Physical requirements:
  
  fine motor skills, lifting up to 35 lbs occupational exposure, etc. (brief note).
