Position Summary

Completion of an ASHP-accredited PGY1 pharmacy residency preferred but not required. Pediatric experience preferred but not required. We are looking for a dedicated inpatient clinical pharmacist to join our team at the Thomas Campus, focusing on supporting the medication needs of our pediatric patients.

This position involves 8-hour evening shifts, offering a unique opportunity to make a meaningful impact during non-traditional hours. Incentives include evening shift differential, relocation assistance for qualified candidates, and a sign-on bonus up to $10,000.

Position Duties

  • Reviews and manages medication orders for therapeutic appropriateness based upon age, weight, height, laboratory values, diagnosis, and other patient and clinical information pertinent to safe and effective care and intervenes as necessary.
  • Effectively screens for and manages clinical alerts/conditions.
  • Actively problem-solves issues with medication orders and contacts the prescriber to recommends appropriate alternative medication therapy as needed.
  • Complies with the directives of the Pharmacy and Therapeutics Committee related to Formulary Management.
  • Documents clinical interventions to meet or exceed established cost containment and evidence-based medication therapy goals for the department and organization.
  • Prepares and dispenses sterile and non-sterile medications, in an accurate and timely manner, in compliance with departmental and organizational policies and procedures.
  • Demonstrates and utilizes appropriate clinical pharmacy skills in assessing and formulating medication therapy recommendations to medical staff and other health care providers.
  • Participates in decentralized Pharmacist assigned duties when applicable.
  • Assists in developing guidelines, policies, and other monitoring and intervention strategies for medication use.
  • Actively participates in the ongoing education and training of self, other pharmacists, pharmacy residents, interns, technicians, and student learners and orientation of new employees.
  • Directly supervises the work of technical, intern, and support staff to ensure accuracy in medication preparation and dispensing.
  • Acts as liaison between the pharmacy and the medical and nursing staff for issues pertaining to medication management and pharmacy services.

Position Qualifications

Bachelor’s Degree (BS) or Doctorate Degree (Pharm.

D.) in Pharmacy or graduating from an accredited school or college of pharmacy in the next 6 months.

  • Completion of a post graduate year 1 (PGY-1) pharmacy residency program accredited by the American Society of Health-System Pharmacists (ASHP).
  • Experience with information systems and automated and technical equipment.
  • One year of experience in a hospital pharmacy practice or equivalent.
  • One year of previous pediatric patient population experience or equivalent.

Special Skills

  • Experience with various types of computer software, including MS Office Suite or equivalent.
  • Ability to problem-solve and collaborate with other team members to support a cohesive work environment.

Additional Requirements

  • Current licensure as a Pharmacist or eligible, either by reciprocity or examination by the Arizona State Board of Pharmacy.
  • Current BLS and PALS or ACLS for Healthcare Provider from the American Heart Association.
