Note:

The Patient Placement Coordinator is the front line of our hospital. You will be the voice, eyes, and ears of our families and providers seeking information about the services Phoenix Children’s can offer. This role requires deep knowledge of internal hospital functions, educational communication, and RN skills to direct patient/physician resources. Incumbent will take incoming calls dealing with highly complex medical cases, requiring a minimum of 4–5 years of Emergency Room or critical care experience, and must hold an RN license.

No new graduates. This is a full‑time, onsite position with 11 AM–12:30 PM appointments (3 12‑hour shifts) and every third weekend.

This position is accountable for coordination and acceptance of information regarding direct admissions and transfers to Phoenix Children’s, coordinating all aspects of patient placement within the facilities, and partnering with nursing and other clinical operation departments to facilitate patient access, throughput, and capacity.

Position Duties
  • Manage all transfer and direct admission requests, facilitate provider communication.
  • Collect intake data from ED, physician office, clinic, and referral facilities.
  • Coordinate MD‑to‑MD acceptance calls, document calls, and maintain quality and customer service.
  • Collaborate with Patient Placement Representative to determine patient benefits and eligibility.
  • Accept all non‑EMS ED referral calls and communicate incoming referrals to ED.
  • Triage and prioritize patient information and acuity using critical thinking and established processes.
  • Show knowledge of human growth, development, and care of patients of all ages.
  • Proactively manage bed placement, throughput, and hospital bed capacity.
  • Maintain bed board accurately and timely.
  • Escalate bed placement and capacity issues to leadership.
  • Work with nursing units and leaders to identify best bed placement considering patient needs, staffing, level of care, infection control, and throughput goals.
  • Plan daily scheduled admissions.
  • Collaborate with admitting, EVS, and case management for timely admissions and positive family experience.
  • Model Family‑Centered Care and Customer Service with positive, timely communication.
  • Use excellent telephone etiquette and clear communication skills.
  • Multi‑task by transcribing information into database while speaking with callers.
  • Participate in department‑specific process improvement, tracking and trending barriers, and recommending solutions.
  • Perform miscellaneous job‑related duties as requested.

Position Qualifications Education
  • Two‑year ADN, four‑year BSN, or Diploma Nursing degree (Required)
  • Bachelor’s degree in Nursing or current enrollment in an RN‑to‑BSN program (Preferred)
Experience
  • Two years clinical RN experience (Required)
  • Pediatric experience (Required)
