Java Backend Developer

We are seeking a compassionate and dedicated Social Worker to support individuals, families, and communities by assessing needs, connecting clients to essential resources, and providing ongoing case management. • Conduct client assessments to evaluate social, emotional, mental health, and support needs.
• Develop individualized care plans and coordinate services to meet client goals.
• Provide crisis intervention, counseling, and support to clients facing difficult life situations.
• Connect clients to community resources such as housing, healthcare, financial aid, and behavioral health services.
• Maintain accurate and timely case notes, reports, and documentation in accordance with state and federal guidelines.
• Collaborate with healthcare providers, schools, law enforcement, and community agencies to ensure holistic support.
• Advocate for clients and help them navigate systems such as Medicaid, social services, and educational support programs.
• Conduct home visits, community outreach, and follow-ups to track client progress.
• Participate in multidisciplinary team meetings and case reviews.
• Ensure compliance with HIPAA, confidentiality laws, and organizational policies.
