• Conduct thorough physical therapy assessments and diagnostic tests, crafting effective care plans based on physician directives.
• Perform initial client evaluations within 48 hours of referral, ensuring meticulous documentation and adherence to agency policies.
• Collaborate with an interdisciplinary team, effectively communicating plans, progress, and modifications to physicians and case managers.
• Provide education and counseling to patients and their families regarding physical therapy needs and available community resources.
• Engage in discharge planning and contribute to quality improvement initiatives, utilizing tools such as OASIS for assessment and outcome analysis.
• Maintain a safe environment for clients and coworkers by adhering to safety and infection control practices.
• Supervise and evaluate home care team members, including aides and therapy assistants, following agency guidelines.

• Graduate of an accredited Physical Therapy program, recognized by pertinent medical and educational authorities.
• Current state license/registration as a Physical Therapist.
• Minimum of two years’ experience in the healthcare field, with a preference for home care experience.
• Current CPR certification and a valid driver’s license with insured automobile.
• Strong communication skills and the ability to work independently.
