Licensed Professional Counselor‑Baseline
Mountain Park Health Center is looking for Licensed Professional Counselors (LPCs) to join our well‑established, accredited team of providers. As a large Federally Qualified Health Center (FQHC) in Arizona, we are dedicated to sustaining and improving health by providing excellent, patient‑centered primary care. In this role, as a Licensed Professional Counselor, you will coordinate with primary medical and behavioral health teams to ensure delivery of comprehensive care to patients.
You will work as an integrated team member in one of our primary care clinics, assessing patients, creating and coordinating treatment plans, and overseeing ongoing treatment. You may also provide traditional counseling/therapy to pediatric and adolescent populations.
- Provide brief, evidence‑based, solution‑focused behavioral interventions with active development of psychotherapeutic/psycho educational behavioral change planning for patients of Mountain Park Health Center.
- Provide quality clinical consultation to patients and providers regarding mental, behavioral, emotional, and physical health‑related issues.
- Provide screening and functional assessment of behavioral health issues for Mountain Park Health Center patients.
- Collaborate with medical providers, support staff, and operational managers to identify patients with medical, cognitive, behavioral, and/or psychological conditions that could benefit from behavioral health interventions, such as motivational interviewing, and provide these services in a timely manner within the clinic setting.
- Develop and implement specific clinical pathways for select populations.
- Facilitate groups to address biopsychosocial issues impacting medical issues.
- Assist with coordination of care and access to behavioral health agencies in the community to meet the patient’s behavioral health care needs with consistent, expeditious feedback to referring providers.
- Provide crisis triage and intervention.
- Help de‑escalate situations with patients who are upset or frustrated.
- Maintain regular and predictable attendance.
- Perform other duties as assigned.
Minimum Qualifications
- Current licensure in the state of Arizona as a Professional Counselor.
- Eligibility for credentialing and contracting with all insurers.
Preferred Qualifications
- Training in Motivational Interviewing (MI).
- Training in Acceptance and Commitment Therapy (ACT).
