Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary

The Community Bridges, Inc. (CBI) Client Care Coordinator is a shelter or temporary lodging site‑based position with a primary role of managing a housing caseload and coordinating care for individuals and families experiencing homelessness. The role involves coordinating care and support services to aid in ending homelessness, complex substance use, mental health, housing, and medical needs, and ensuring coordination with existing PCMH or PCP services.

The Coordinator serves as the bridge for residents, coordinating and monitoring RBHA and Non‑RBHA community‑based services and facilitating delivery. The position works with individuals and families of the highest acuity and vulnerability, coordinating through the CBI Continuum of Care or other community agencies to address social determinants of health. Additionally, the Coordinator completes data entry, reports, and end‑of‑shift documentation to support program outcomes.

Skills

/ Requirements
  • High school diploma or GED required;
    Associate degree or higher in a field related to Behavioral Health preferred.
  • Minimum of twelve months of recovery from substance use and/or mental health disorders required.
  • Minimum of six (6) months of paid related work experience providing exposure to behavioral and medical crisis situations required.
  • 1–3 years of full‑time Health Care related work experience providing exposure to behavioral and medical crisis situations preferred.
