In this role, you will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs work in conjunction with dialysis RNs to provide dialysis care.
- Spending the majority of your day in direct one‑on‑one patient care to provide safe, comfortable and hygienic dialysis treatment.
- Handling the complexity of DSD Dialysis; meeting and/or exceeding clinical outcomes while multitasking to meet the needs of patients and our SNF partners.
- Maintaining autonomy and critical thinking: monitor patients before, during and after dialysis, measuring and recording stats, patient observations, and hemodialysis machine setup.
- Provide comprehensive patient care in an independent setting.
- Plan for continuous monitoring of patients before, during and after dialysis.
- Collaborate with cross‑functional resources in the Village.
- High school diploma or equivalent.
- CCHT certified.
- Basic computer skills and proficiency in MS Word and Outlook.
- Experience with dialysis (preferred).
- Background in training, teaching or healthcare education.
