Terros Health is hiring a Full-Time Health Navigator in the Phoenix Metro Area.

High School Diploma or GED in Combination With One (1) Year of Behavioral Health Experience.
Schedule:Full-time Monday
- Friday, Daytime Shift Flexible to Work Weekends & Evenings

The Health Navigator will work within our Federally Qualified Health Centers (FQHCs) to support patients in accessing and navigating healthcare services with a focus on the Medication to Address Opioid Use Disorders (MOUD) program and closing essential gaps in care. This role is essential in improving health outcomes by providing education, resources, and guidance for underserved populations, including adults with Serious Mental Illness, uninsured, underinsured, and vulnerable community members.
The Health Navigator collaborates with healthcare providers, case managers, and external community resources to promote patient-centered care and facilitate access to essential health services. The Health Navigator will provide BHT level behavioral health and skills interventions to facilitate overall wellness and health to patients served. This position reports to the Health Center Director or Clinical Site Manager.
- Provides outreach, engagement, care coordination, and patient navigation services within Mobile Health programs.
- Conducts outreach in shelters, schools, community centers, outreach locations, community events, and other community-based settings.
- Builds trusting relationships with patients and community members to improve healthcare engagement.
- Assists patients in accessing primary care, behavioral health, substance use treatment, specialty care, and community resources.
- Identifies potential barriers to quality care and opportunities for improvement and reports them to the Operational Supervisor and/or the Director of Nursing.
- Coordinates referrals and tracks completion of follow-up services.
- Ensures documentation of patient encounters in accordance with policies, procedures, and clinical protocols same date of service.
- Supports transitions of care following emergency department visits, hospitalizations, and crisis encounters.
- Conducts Social Determinants of Health screenings and assists patients with housing, transportation, food assistance, insurance, employment, and other support services.
