Responsibilities
The Inpatient & Outpatient Servicesdepartments arecomprisedof
Clinicians (Social Workers/Therapists),nurses,psychiatrists,receptionists, Program Manager Care Coordination, andstepdownnavigator.
The program manager care coordinationassiststheinpatient and outpatientclinicalteamwith coordination of carefor patients that are inpatient or outpatient at Valley Hospital and Calvary Healing Center. This personpromotes Outpatient Services across 5 locations in the Phoenix area, being the primary connection for our sober living partners and the patients participating in the program.
The program manager care coordinatorisresponsible forhaving a thorough understanding of the Outpatient Programs andengagingeffectively with the Inpatient Clinical Services department to promote Outpatient Programs.
The program manager care coordinationwill engage with patients (inpatient & outpatient) dailytoidentifytheir needs and discuss how Outpatient Services &/or sober living will be beneficial for them.
This person will manage the tracking and communication for care coordination between the facilities and the sober living environments which includes 150+ beds.
This person advocates for the patient’s access to services during treatment team meetings, communication with outpatientteams, housing partners,and referrals.
They are responsible for complying with UHS legal expectations related to housing scholarship applications, consent forms, and complying with AZ Department of health & Joint Commission/CARF regulations.
Requirements:
- Bachelor's Degree from an accredited college or university in behavioral health, or related healthcare;
- Three (3) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
- Successful completion of CPR Certification prior to hire
- Handle with care
- May be required to work overtime and flexible hours.
