Occupational Therapist, Registered; OTR

We are currently seeking a Full-Time Registered Occupational Therapist (OTR) to join our skilled nursing team. The ideal candidate is compassionate, organized, and committed to helping residents regain independence and improve their quality of life.

** Must be willing to relocate to Phoenix, AZ

• Conduct occupational therapy evaluations and develop individualized treatment plans
• Provide skilled OT services to residents in a skilled nursing setting
• Assist residents with activities of daily living (ADLs), mobility, and functional independence
• Collaborate with an interdisciplinary team to support resident-centered care
• Complete timely and accurate documentation in accordance with Medicare and facility guidelines
• Participate in care planning and discharge planning as needed

• Active Occupational Therapist (OTR) license in the state of practice
• Graduate of an accredited Occupational Therapy program
• SNF or post-acute experience preferred
• Strong communication and documentation skills
• Ability to manage a caseload effectively in a fast-paced environment
