- You will oversee the intake and case management staff for Phoenix Rescue Mission clinical and residential programs, ensuring that clients receive comprehensive, coordinated care.
- You will develop, implement, and oversee case management policies, procedures, and protocols to ensure high‑quality, client‑centered care.
- You will provide religious guidance to clients and show an openness to discussing Biblical principles, as appropriate in non‑billable services.
- You will partner with clinical staff, administration, and other departments to coordinate care.
- You will monitor case management activities to ensure that clients receive timely, comprehensive, coordinated care through effective case management practices.
- You will address and resolve client issues or barriers to care, providing support and advocacy as needed.
- You will develop and maintain performance metrics to monitor the effectiveness of case management services and ensure that desired outcomes are being achieved.
- You will develop and maintain relationships with external partners, including healthcare providers, community organizations, and referral sources.
- You will foster a climate consistent with the Mission’s core values.
- You have a bachelor’s or master’s degree in Social Work, Psychology, Counseling, Nursing, or related Human Services field and at least three years of experience in behavioral health case management or a combination of education and experience.
- You have at least five years of management experience, resulting in strong demonstrated leadership, administrative, and supervisory skills.
- You have a strong knowledge of case management principles, behavioral health issues, and evidence‑based practices.
- You are proficient in Microsoft Office Suite, electronic health records, and other software.
- You have a solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions, reflecting Christ’s unconditional love and advocacy to provide the highest quality care to clients, and a deep commitment to Christian values and the mission of faith‑based recovery.
- You have strong initiative, self‑direction, flexibility, and ability to complete tasks in a timely manner.
- You have excellent organizational, communication, and interpersonal skills.
- You have the ability to work effectively in a fast‑paced and dynamic environment.
- You have the ability to work a flexible schedule as the position will require weekend and evening responsibilities as well as on‑call responsibilities.
