Pathway of Hope Coordinator

Location: Southwest DHQ

Employment Type: Full-time (FT – 40hrs/week)

Job Summary

The Divisional Pathway of Hope (POH) Coordinator will coordinate the Southwest Divisions POH approach. The POH Coordinator is responsible for supervising and supporting services offered to families through the POH case management approach at identified Corps or Social Service Sites. The POH Coordinator will work collaboratively with the Divisional Social Service Director and with field units to support the implementation of the POH approach in the SW Division, including staff training/consultation, quality assurance, program evaluation, and outcome reporting.

Qualifications
  • Bachelor of Social Work (BSW) or comparable degree from an accredited college or university required
  • For a BSW, a minimum of six years of experience in supervision of social service programs providing case management with at‑risk, economically, and socially vulnerable populations.
  • Master of Social Work (MSW) or comparable degree from an accredited college or university preferred
  • For an MSW, a minimum of four years of experience in supervision of social service programs providing case management with at‑risk, economically, and socially vulnerable populations.
  • Demonstrated capacity in building collaborative community service program models.
  • Ability to work with minimal supervision while maintaining a high level of productivity
  • Must be able to create positive working relationships with economically, educationally, & socially diverse populations
  • Familiarity with client databases and computer‑generated reports.
  • Demonstrated capacity to teach adults.
  • Ability to maintain a self‑motivated and self‑directed schedule, address multiple projects in a timely manner and take projects from beginning through completion.
  • Familiarity with Salvation Army policies and procedures.
  • Excellent verbal and written communication skills.
  • Computer literate in MS Office software applications.
  • Must be able to pass a criminal background screening.
  • Travel required, including nights and weekends.
  • Must have a valid Arizona DL, with a clean driving record and pass The Salvation Army’s Fleet Safety Training.
Responsibilities Planning and Implementation
  • Ensure quality of both POH planning & implementation by participation with POH planning & program development at sites.
  • Ensure consistent communication by participating in regularly scheduled conference calls with the Territorial Pathway of Hope Project Manager, local caseworkers, officers, and the DSSD.
  • Ensure consistent internal communication and understanding of program developments by sharing electronic and hard copies of updated information with divisional staff and corps. Additionally, prepare bi‑monthly reports on the POH progress and status for the Territorial POH Project Manager and DSSD.
  • Ensure quality of local data and reporting of information that will support national and territorial POH program outcomes.
  • Ensure relevance and consistency of POH by collaborating with the Territorial Project Manager and DSSD with ongoing development of policies and procedures, goals and objectives, and outcome‑based measures related to the POH.
  • Ensure relevance of POH approach to funders by keeping current on program funding resources, both public and private, and assist DSSD and Development staff to support local leadership in securing additional localized funding.
  • Assist scaling of POH and learning agenda by collaborating with the Territorial POH Project Manager, the DSSD and other organizations for the purposes of further development of the POH approach and support kindred projects, as requested and/or assigned.
Evaluation and Outcome Measurement
  • Ensure the accuracy of data entry into the Well Sky Community Services database.
