Nurse Practitioner or Clinical Nurse Specialist - Transitional Care

Responsibilities
  • Conduct comprehensive patient assessments, including medical history, physical examination, and review of symptoms
  • Order and interpret diagnostic tests, such as laboratory work and imaging studies
  • Diagnose and treat acute and chronic illnesses, injuries, and conditions
  • Develop and implement patient care plans in collaboration with the healthcare team
  • Prescribe medications and other treatments as necessary
  • Provide education and counseling to patients and their families on health promotion, disease prevention, and treatment options
  • Perform procedures such as wound care, suturing, and administering injections
  • Collaborate with other healthcare professionals to coordinate patient care
  • Maintain accurate and up-to-date medical records
Qualifications
  • Valid Nurse Practitioner license in the state of practice
  • Master's degree in Nursing with specialization as a Nurse Practitioner
  • Experience working as a Nurse Practitioner preferred
  • Strong knowledge of physiology and pathophysiology
  • Proficiency in conducting physical examinations and interpreting diagnostic tests
  • Ability to develop and implement patient care plans based on assessment findings
  • Excellent communication skills to effectively educate patients and collaborate with the healthcare team
  • Familiarity with utilization review processes and procedures
  • Experience in dermal fillers administration is a plus
  • Knowledge of triage protocols for emergency situations
  • Competency in specimen collection processing for laboratory testing
  • Ability to perform procedures such as ventilating patients or performing spinal taps is desired but not required
