The RN Navigator for the Pulmonary Hypertension program in our Advanced Lung Disease Clinic at BUMCP leverages their clinical knowledge, experience, critical thinking, and nursing judgement to support and coordinate the complex needs of patients. The Nurse Navigator is continuously assessing for barriers to care and addressing them by providing education, advocacy, care coordination and supporting continuity of care, allowing patients and caregivers to make informed decisions about their healthcare journey.

This role supports the patients’ specialty journey needs including coordination of care, testing and multidisciplinary follow-up, providing specialty education and making 24-48 pre- and post- calls, assessing and addressing any barriers, triaging complications and follow-up needs; further developing the relationship and connection of Sophia to Banner Health.

Minimum 3 years of clinical nursing experience, with at least 2 years in pulmonology, or chronic disease management. Other background that would assist in being successful in this role:
Ambulatory/Clinic, Navigation, Care Coordination, Med Surg, PCU, process improvement, patient quality and safety. Excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination preferred.

This is a full time (40 hours/week), salaried, day shift position. The average day starts at 0800 and ends at 1600 and is Monday through Friday. With this role, hours are dependent on patient needs and can start earlier or later on occasion. We believe in a work-life balance and will adjust accordingly if the need arises.

POSITION SUMMARY  This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
