Licensed Primary Care Provider (Nurse Practitioner)

Inclusi Care Medical is seeking a dedicated nurse practitioner to join our team as an Independent Contractor to provide patient-centered clinical care. This role is flexible, allowing for in-person or telemedicine visits, and is designed for providers who want to serve diverse communities while maintaining autonomy.

Contractor agrees to perform services in a manner consistent with professional standards, state regulations, and Inclusi Care’s clinical quality requirements as applicable to contracted services. At Inclusi Care Medical, our contractors may build an independent patient panel over time. As such, we emphasize the importance of patience and persistence while the patient base grows. This approach supports long-term success and fosters meaningful provider–patient relationships over time.

• Compensation
  : $20-$25.00 (experienced-based) per billable visit in the first 12 months. Rate subject to review after this period; negotiable.

• Clinical Teaching
  :
  Opportunities to precept PA students. Preceptors receive a majority of the paid clinical placement fee from student programs as a stipend based on time served.

• VPN access provided by Inclusi Care Medical.
• Providers use their own devices in compliance with Inclusi Care’s IT security standards (BYOD policy), if working from home.

• Professional liability insurance is provided by Inclusi Care Medical, but the applicant must be licensed in Arizona and board-certified.

• Board-certified NP with active Arizona license (or eligible).
• Comfortable with telemedicine care or in-clinic care.
• Strong commitment to providing inclusive, high-quality care.
• Interest in clinical teaching and mentoring students is highly valued.

This posting is for independent contractor positions only. The applicant understands they will act solely as an independent contractor of Inclusi Care Medical and not as an employee, partner, joint venturer, or agent. The independent contractor status shall be solely responsible for payment of all federal, state, and local taxes arising from compensation received for contracted services.

Selected independent contractor providers should anticipate a credentialing period of approximately 90-120 days for Inclusi Care Medical to secure enrollment with the necessary insurance payers to support standard clinical operations.

Selected independent contractor providers should anticipate a gradual increase in patient volume when joining Inclusi Care Medical. As a new and expanding practice, patient panels are still developing, and providers should expect an initial period of modest volume with the opportunity to actively build and grow their individual practice over time.
