Pride Health is hiring a Home Health RN to support our client’s medical facility in Phoenix, AZ. This is a 13-week assignment (with the Possibility of extension or conversion) and a great opportunity to start working with a top-tier healthcare organization!

Title: Home Health RN

Location: Phoenix, AZ

Schedule: 5x8 Days

Contract Length: 13 Weeks with possible extension.

The Registered Nurse (RN) provides skilled nursing care to patients of all ages in their homes. This role is responsible for assessing patient needs, developing and implementing individualized Plans of Care, coordinating services with the interdisciplinary team, and ensuring high-quality, compliant patient care. The RN educates patients and families, manages pain and treatments, supervises home health staff, and maintains accurate documentation in accordance with regulatory and payer requirements.

On-call duties and local travel are required.

• Conduct comprehensive patient assessments in stable and emergent situations
• Develop, implement, and evaluate Plans of Care (POC)
• Provide skilled nursing care, wound care, IV therapy, medication administration, and pain management
• Educate patients and families on disease management, medications, safety, and self-care
• Ensure accurate and timely documentation and regulatory compliance
• Supervise and guide HHAs/STNAs/LVNs
• Coordinate referrals and communicate with physicians and care teams
• Participate in interdisciplinary meetings and on-call rotations.

• Active state RN license and CPR certification
• 1–2 years RN clinical experience; 2+ years home health experience required
• OASIS and Homecare Homebase experience required
• Knowledge of IV therapy, wound care, catheters, and home medical equipment
• Valid driver’s license with reliable transportation
• Strong communication, organization, and independent work skills.

• Associate Degree in Nursing or Nursing Diploma required
• BSN preferred
