POSITION

PURPOSE

The In‑Home Assessment Nurse Practitioner (IHA NP) assesses patient and family psychosocial needs, plans and implements services, and evaluates care while meeting members in their homes. Your goal is to improve quality of life and health literacy by conducting in‑home visits (up to one hour) that evaluate medical history, medications, social determinants, and risks. The IHA NP coordinates, monitors, and ensures timely information sharing across the continuum of care and promotes effective use of tools and resources.

Both part‑time and full‑time opportunities are available. This position requires travel and mileage reimbursement is provided.

DUTIES & RESPONSIBILITIES
  • Travel up to 70% throughout designated regions.
  • Conduct in‑home assessments following established guidelines.
  • Formulate a list of current and past medical conditions using clinical knowledge and findings.
  • Identify urgent and emergent situations for proper intervention.
  • Educate members on health conditions and continuity of care.
  • Identify diagnoses for care management and active medical management.
  • Provide care coordination to alleviate identified behavioral, physical, and social issues.
  • Focus on addressing issues that will decrease medical costs and increase quality of life.
  • Develop positive relationships with clients, caregivers, providers, and team members.
  • Manage physical health problems through education.
  • Communicate gaps in care to the member’s primary care physician if needed.
  • Conduct home safety evaluations.
  • Identify internal and external referral needs when available.
  • Maintain HIPAA compliance and protect PHI.
SKILLS & QUALIFICATION
  • One (1) year of NP experience is preferred.
  • Experience caring for the elderly in some regions is preferred.
  • Experience in home health, physician’s office, or public health is preferred.
  • Computer skills required:
    Microsoft Office Suite.
  • Adapt to changes in policies, procedures, new techniques, and additional responsibilities.
  • Good driving record.
  • Knowledge of HIPAA regulations.
  • Bilingual (English/Spanish) is preferred.
EDUCATION, LICENSES, & CERTIFICATIONS
  • Master’s Degree in Nursing is required.
  • Current Licensed Nurse Practitioner in Arizona is required.
  • Current CPR certification is required.
  • Annual TB testing is required.
