Position Overview

We are seeking compassionate and experienced Home Health RNs to join our team. In this role, you will provide skilled nursing care to patients in their homes, assessing patient conditions, administering treatments, and collaborating with physicians and other healthcare professionals to ensure the best possible outcomes. Your expertise and care will directly impact patients’ recovery and overall well-being.

Key Responsibilities
  • Perform comprehensive patient assessments and develop individualized care plans in collaboration with physicians.
  • Administer medications, wound care, and other nursing treatments in a home setting.
  • Monitor patient progress and modify care plans as needed to promote recovery and comfort.
  • Educate patients and their families on managing medical conditions, medications, and home care techniques.
  • Provide health assessments, coordinate care, and ensure effective communication with the patient’s care team.
  • Ensure patient safety and implement proper infection control and health care practices.
  • Document all patient visits, treatments, and progress in compliance with regulatory and company policies.
  • Advocate for patient needs and ensure continuity of care through collaboration with interdisciplinary teams.
  • Offer emotional support and guidance to patients and families, fostering a trusting and empathetic relationship.
Qualifications
  • Current and active license in Arizona.
  • 2 years of nursing experience, with at least 1 year in home health / hospice.
  • Strong knowledge of clinical nursing practices, including patient assessments, care plans, and treatment protocols.
  • Excellent communication skills and the ability to work effectively with patients, families, and multidisciplinary teams.
  • Ability to work independently and manage multiple patients and tasks efficiently.
  • Valid driver’s license and reliable transportation for home visits.
  • Compassionate, detail-oriented, and committed to patient care.
