Position Overview:We are seeking a Full-Time Nurse Practitioner (NP) to join our dynamic and supportive team. The ideal candidate is committed to providing high-quality, evidence-based care and is passionate about working with diverse populations. Key Responsibilities:- Perform patient assessments, physical exams, and diagnostic evaluations
- Diagnose and treat acute and chronic health conditions
- Prescribe medications and manage treatment plans
- Educate patients on health promotion and disease prevention
- Collaborate with physicians, behavioral health providers, and care coordinators
- Participate in chronic care management and MAT services as appropriate
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Engage in ongoing quality improvement and team-based care initiatives
- Current and unrestricted Nurse Practitioner license in Arizona, or eligible for licensure in AZ.
- Board certification as a Family or Adult NP (FNP, ANP, or AGNP)
- DEA license and prescriptive authority
- Minimum of 1–2 years of primary care experience preferred (new grads considered)
- Experience with MAT, behavioral health, or chronic care is a plus
- Strong communication and clinical skills
- Proficiency with EHR systems (eClinicalspreferred)
- Bilingual (Spanish, French, or other languages) a plus
