The Position:Using strong clinical and assessment skills to assess patients’ post-acute needs in collaboration with the multi-disciplinary Halos Home Team and the patients discharge care providers to ensure continuity of care when discharging home.
Essential Duties and Responsibilities (Other duties may be assigned):- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel
