Additional Language Differential Pay Available:
Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL: $3/hour

The Primary Care Provider conducts physicals/check-ups, medical evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, makes specialty care referrals, educates patients about preventative care, establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate treatment goals, within the context of an integrated setting.

Duties include:

Performs independently or as a part of a treatment team.

Conducts physicals/check-ups, evaluations, diagnostics, treats illnesses, orders lab tests, general and preventative care, medication prescribing and monitoring, and individual treatment and case consultation.

Performs the important task of educating patients about preventative care, wellness activities and prescribed treatments.

Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non- professional medical staff, and students.

Participates in program evaluation procedures and professional record keeping, makes referrals appropriately for specialty and behavioral health care.

Provides direct patient services not less than 85% of the work time Employs therapeutic interventions appropriate for identified treatment issues

Seeks appropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment

Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations

Possess a valid BLS/CPR card Possess a valid DEA license

Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy

Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level
1) within 7 working days of assuming role.

Must pass a background check, drug screen and TB test.

Proficient using computer-based systems including electronic health records (Next Gen preferred), Microsoft Outlook, and other Microsoft Office products.
