Monday - Friday Shift - 2:00pm to 10:00pm

• Conduct initial assessments of potential clients, gathering all relevant information including a comprehensive inspection, inventory, and documentation of the client’s possessions.
• Input accurate and complete client data into the Mission’s database.
• Coordinate the delivery of support services for clients (medical, legal, ancillary) to remove obstacles and enhance client success.
• Handle emergencies and crises as they arise, following established protocols.
• Conduct case management assessments on community, shelter, and program clients, and assist with proper program placement.
• Transport clients when necessary.
• Distribute medication as needed.
• Comply with all relevant legal, regulatory, and compliance requirements, company policies and procedures, and the Code of Ethical Conduct.
• Follow the Mission’s Statement of Faith and lifestyle and adhere to the employee policy manual and guidelines set forth by the CEO.
• Lead or participate in devotions, prayers or other organization activities that promote a credible Christian witness and lifestyle.
• Perform other duties within scope as assigned.

• Bachelor’s degree in social work, sociology, psychology, counseling, or related social service field; or 4 years of experience in case management or related field; or an equivalent combination of education and experience.
• Certification in Peer Support highly desired.
• Proficient in Microsoft Office software.
• Program database experience preferred.
