Base pay range$21.00/hr - $23.00/hr
Case Manager - Children's Behavioral Health: JFCS is seeking a compassionate professional to provide case‑management services to children receiving behavioral‑health services across The Valley. The Child Case Manager also provides guidance and support to the families/caregivers of these youth as needed.
Duties include:
- Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
- Assists youth in navigating discharge from treatment settings, in finding services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
- Coordinates services and collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
- Coordinates and facilitates Child & Family Team (CFT) Meetings.
- Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
Qualifications:
Meets one of the following education/experience requirements:
- High School Diploma/GED required and 4 years of Behavioral Health experience.
- A.A. Degree and two years of Behavioral Health experience
- Bachelor’s Degree in Social Work, Psychology, or related field.
- Level 1 Fingerprint Clearance Card without Restriction(s)
- Meet Agency requirements for MVR (Motor Vehicle Record) report
- Reliable transportation
- AZ driver’s license
