Job Summary

Case managers assist transitional housing residents in accessing care and services, advocating for resident needs, and assist in removing barriers to accessing permanent housing for residents. The case manager works with assigned residents to develop housing plans and assists residents toward income and housing goals.

The ideal candidate enjoys working as part of a team of staff and volunteers and can think and act quickly when there is a need to pivot or address an emergency. This candidate can think critically and help guests determine a course of action that will move them toward stability. This individual can work independently while ensuring complete and thorough communication with team members.

The best candidate for this post is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow-through from residents. This representative must be energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations. This person will exhibit the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment.

The goal of this position is to provide program residents with the tools and support required to permanently end their homelessness.

Specific Duties
  • Carries caseload of up to 25 residents, meeting with each resident a minimum of once a week.
  • Completes intake appointments with prospective residents, including gathering background information and writing intake assessments.
  • With input from residents, develops and monitors progress on service plans.
  • Maintains timely and accurate records in case file and HMIS, and UPLIFT database, including contacts with resident, family members and social agencies, etc.
  • Participate in weekly case conferencing to monitor resident progress, problem solve with peers and share resources.
  • Maintains timely and thorough case notes and completes monthly reports for Program Manager.
  • Coordinates with other program staff as needed regarding resident interaction within the program.
  • Assists residents with navigating systems in the community, including attending appointments as needed and providing hands on assistance to overcoming barriers.
  • Coordinates and advocates with referral agencies to facilitate appropriate services.
  • Mediates and facilitates problem solving with residents, including conducting group meetings or individual coaching.
  • Other duties as assigned by Program Manager

  • Bachelor's degree OR minimum 4+ years of progress experience in social services or related field
  • High School Diploma, or GED (minimum education)
  • Must have valid driver’s license and ability to provide 39 month MVR (motor vehicle record) showing clean driving habits
  • Fingerprint Clearance Card or ability to obtain within first 90 days of employment
  • Abilities to stand and walk, lift up to 50 pounds, sit, bend, and reach are required for this position.
  • Ability to read, comprehend, and communicate orally and in writing required. (English)
Preferred Skills/Abilities
  • Excellent communication, organization skills, and ability to interact with guests, staff, and volunteers required.
  • Related experience working with vulnerable populations in a transitional housing or shelter environment preferred.
