About UMOM New Day Centers:Founded in 1964, UMOM is Arizona's largest provider of services for families experiencing homelessness. Our mission is to end family homelessness by restoring hope and rebuilding lives. With an annual operating budget of $20 million dollars, UMOM provides shelter, supportive services, and housing programs to families and individuals experiencing homelessness in Maricopa County. UMOM serves over 15,000 unique individuals across all our services annually, including those families receiving support at the Family Housing Hub, Maricopa County's family coordinated entry system.
PositionDescription:
The SSVF Case Manager is responsible for providing comprehensive, housing-focused case management to Veterans and Veteran families participating in UMOM's Supportive Services for Veteran Families (SSVF) program. This position works directly with Veterans to identify and address barriers to housing stability, develop individualized housing and service plans, connect participants with income and community resources, and support participants in obtaining and maintaining permanent housing.
The SSVF Case Manager builds strong, respectful relationships with Veterans and collaborates with landlords, community providers, Veteran-serving organizations, and internal teams to coordinate services and promote long-term housing stability.
Client Engagement and Rapport Building
- Engage Veterans and Veteran families in a respectful, client-centered manner to build rapport and establish a collaborative case management relationship.
- Assess immediate needs, housing barriers, strengths, income, benefits, and other factors impacting housing stability while identifying and strategizing barriers to long-term success.
- Orient participants to SSVF program expectations, available services, and applicable resources.
- Utilize Trauma Informed Care, Motivational Interviewing, and other evidence-based approaches when providing services.
- Work collaboratively with Veterans to develop individualized case plans focused on obtaining and maintaining permanent housing.
- Coordinate services with internal UMOM teams and external providers to ensure Veterans receive comprehensive support.
- Develop and maintain collaborative relationships with community providers and organizations serving Veterans.
- Participate in case staffing and service coordination meetings to identify solutions to participant barriers and strengthen housing stability plans.
- Maintains a friendly, accessible, approachable, and responsive attitude to the needs of the clients on your case load.
- Maintain timely, accurate, and complete participant records, case notes, assessments, and other required documentation in accordance with SSVF, funder, HMIS, and UMOM requirements and maintain participant confidentiality and comply with applicable program and agency standards.
- Utilizes evidence-based practices in service provision, including but not limited to, Motivational Interviewing and Trauma Informed Care.
- Supports clients with securing identification and documentation needed to access housing.
- Assists clients in co-creating a case plan to be able to achieve permanent housing based on their unique needs and circumstances and regularly meet to discuss and review progress.
- Meets regularly and consistently with your client to hold structured, intentional conversations focused on the skill-building necessary to obtain and retain employment/regular income and obtain and retain permanent housing.
- Transport and/or accompany clients as needed to support with housing and/or employment search or to make connections to other resources.
- Ensures clients exiting to permanent housing are supported with an orientation to the neighborhood and that connections are made with local contacts and resources, inclusive of but not limited to local viable transportation options.

Experience and Education
- High School or GED Diploma, required
- Bachelor's degree, preferred
- Minimum 3 years of experience working with at-risk populations and/or case management required
- Demonstrated knowledge of Motivational Interviewing and Trauma Informed Care preferred
