UMOM New Day Centers in Phoenix seeks an experienced SSVF Case Manager to provide comprehensive, housing-focused case management to Veterans and veteran families in the SSVF program. You will identify barriers to housing stability and develop individualized housing plans.
You will coordinate with UMOM teams and community providers, maintain confidential records, and promote long-term housing stability through trauma-informed care and motivational interviewing.
