U.S.VETS Phoenix is hiring a Case Manager I to guide veterans toward independence. Under supervision, you assess needs across housing, employment, education, finances, health, and legal matters, coordinating services with partners and VA staff.
You’ll prepare IAPs, maintain confidential records, and document progress in DAP format. The role involves home visits, data entry into HMIS, and contributing to quality management activities.
