At Extendicare, you'll play a crucial role in assessing, planning, and implementing care plans that reflect the unique needs of residents. Your responsibilities will include administering care and medications, facilitating care conferences, and maintaining accurate documentation. Collaboration with the Director of Care and interdisciplinary team is essential, ensuring seamless transitions for residents.

Key Responsibilities:

• Perform residents' clinical care assessments and care plans

• Administer medications and document in compliance with standards

• Coordinate nursing team care assignments for efficiency

• Provide education to residents and families

• Manage medical emergencies and respond to changes in condition
Requirements:

• Active registration as Registered Practical Nurse

• Experience in seniors' environment preferred

• Strong communication and critical thinking skills

• Current Basic Cardiac Life Support certification

• Computer proficiency in Microsoft Office
