What is a Patient Care Coordinator?

  • Entry‑level office role that maintains pleasant and consistent daily operations of the clinic.
  • Excellent customer service skills.
  • Multitasks in multiple computer programs each day.

A day in the life of a Patient Care Coordinator:

  • Greets everyone who enters the clinic in a friendly and welcoming manner.
  • Schedules new referrals received by fax or telephone from patients and physician offices.
  • Verifies insurance coverage for patients.
  • Collects patient payments.
  • Maintains an orderly and organized front office workspace.
  • Other duties as assigned.

Position Summary:

The Patient Care Coordinator - I (PCC‑I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC‑I position supports the mission, vision, and values of Upstream Rehabilitation.

Responsibilities:

  • Collect all money due at the time of service.
  • Convert referrals into evaluations.
  • Schedule patient visits.
  • Customer Service.
  • Create an inviting clinic atmosphere.
  • Make all welcome calls.
  • Monitor and influence arrival rate through creation of a great customer experience.
  • Manage schedule efficiently.
  • Manage document routing.
  • Manage personal overtime.
  • Manage non‑clinical documentation.
  • Manage deposits.
  • Manage caseload, D/C candidate, progress note, and insurance reporting.
  • Monitor clinic inventory.
  • Attend required training with the Territory Field Trainers (TFT) for Raintree and other business process updates.
  • Complete quarterly compliance training.

Qualifications:

  • High School Diploma or equivalent.
  • Communication skills – must be able to relate well to Business Office and Field leadership.
  • Ability to multitask, organizational detail, ability to meet deadlines, work with little to no supervision.
  • As a member of a team, must possess efficient time management and presentation skills.

Physical Requirements:

  • Subject to inside environmental conditions; exposed to noise consistent with indoor environment.
  • Full‑time position operating within normal business hours Monday through Friday; may be required to attend special events some evenings and weekends, or work additional hours as needed.
  • Subject to sedentary work; constant sitting with ability to interchange with standing.
  • Constantly communicates with associates; must be able to hear and speak accurately.
  • Frequently operates a computer and other office equipment using gross and fine manipulation; constant repetitive typing; must be able to view computer screen and read items on screen.
  • May occasionally walk, stand, push or pull 0‑20 lbs., lift 0‑20 lbs. from floor to waist; carry, push, and pull 0‑20 lbs.
  • Rarely crawls, crouches, kneels, stoops, climbs stairs or ladders, reaches above shoulder height, lifts under 10 lbs. from waist to shoulder.
