Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served.

• Evaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine the levels of functioning and intervention needs
• Demonstrates knowledge of age‑specific factors and performs satisfactorily related to patient’s age infant through geriatrics as noted on unit specific guidelines and competencies
• Develops and implements individualized treatment programs for the patients needing services
• Establishes and modifies intervention goals and strategies for patients/ treatment program based on ongoing evaluations. Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals
• Provides direct therapy services to patients using various functional settings and strategies
• Plans therapy with in the patient’s individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans
• Prepares written documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports
• Communicates effectively with patient, family, physician, and other disciplinary team members regarding the patient status, progress, and needs
• Establishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient’s intervention program
• Participates in program development of the department. Evaluates and monitors clinical internship with students affiliating in various universities/colleges

• CCC‑SLP – American Speech‑Language‑Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment upon hire
• Speech‑Language Pathologist – Licensing Board – an active license in the state of practice upon hire

• 1‑3 years Speech‑Language Pathologist in a hospital, SNF, outpatient and/or home health setting
