• Conduct comprehensive assessments of patients' physical abilities, mobility, and rehabilitation needs in their home environment.
• Develop and implement personalized treatment plans tailored to each patient's specific goals and conditions.
• Provide hands‑on therapy techniques, including exercises, manual therapy, and modalities to improve patients' strength, balance, and coordination.
• Educate patients and their families on exercises, safety measures, and home modifications to facilitate recovery.
• Collaborate with other healthcare professionals to ensure a holistic approach to patient care.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.
• Stay updated on best practices and advancements in physical therapy and home health care.

• A degree in Physical Therapy from an accredited school/program.
• A current, unrestricted Maryland license as a Physical Therapist.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
• Experience in Home Health Therapy and completion of OASIS documentation is preferred.
