PROLOGUE JOB DESCRIPTION
Position Title:
Counselor - Psychiatric Rehabilitation Program (PRP)
General Description:
The PRP Counselor provides daily services to adults recovering from mental illness and/or substance use disorders. The PRP Counselor will work directly with individuals in developing service plans, providing psychoeducation, and providing skills training. The PRP Counselor works with individuals, 1:1 and in groups to assist them in choosing and obtaining personal goals and developing the skills necessary for independence and success in their environment of choice.
Schedule:
Full time, 40 hours weekly. This person reports to the Program Coordinator. FLSA:
Non-exempt.
Note:

- Complete contact notes, Individual Service Plans, Monthly Progress Notes, and other documents as needed and/or assigned.
- Assist individuals on goals and interventions as stated on the Individual Service Plan.
- Provide day program coverage (on-site services), facilitate groups, and develop group lesson plans.
- Carry caseload of individuals for rehabilitation.
- Ongoing communication with rehabilitation team, including Case Manager, Residential Rehabilitation Program, Health Home, and Supported Employment to ensure continuum of care.
- Provide off site services in environments of choice such as in the individual s home or in the community.
- Perform medication management as needed.
- Facilitate client appointments and complete follow-up paperwork.
- Utilize Electronic Health Record to record services and documentation.
- Utilize computer and systems for external and internal communication, time and attendance.
- Other duties as assigned by program supervisors.
Physical Requirements
- Drive Prologue vehicles.
- Assist clients with attending appointments (somatic, psychiatric, etc.).
- Physically assist clients with their personal mobility by working with wheelchairs, assisting with walkers, etc.
- Required Education, Knowledge and Skills to Perform
Essential Job Functions:
An associate s or bachelor s degree is preferred. - Ability to work as a team player is necessary.
- A good driving record is required and is defined as having a valid driver s license with no more than 2 points.
- The ability to respect individual choice in the recovery process.
- Good verbal and written communication skills and intermediate computer literacy.
