Occupational Therapist home health PRN

We are looking for a dedicated Home Health Occupational Therapist to join our team. The ideal candidate will provide personalized occupational therapy services to patients in their homes, helping our clients achieve greater independence and improve their quality of life.

Responsibilities:

• Conduct thorough evaluations of patients' functional abilities, daily living skills, and rehabilitation needs in their home environment.
• Develop and implement individualized treatment plans that address each patient's specific goals and challenges.
• Provide therapeutic interventions to enhance patients' motor skills, cognitive functioning, and adaptive techniques for daily activities.
• Educate patients and their families on strategies to promote independence, safety, and optimal functioning at home.
• Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care.
• Document patient progress, treatment plans, and outcomes accurately and in a timely manner.
• Stay informed about the latest developments in occupational therapy and home health care practices.

Qualifications:

• Experience in Home Health Therapy
• Bachelor's degree in Occupational Therapy from an accredited school.
• CPR
• 2 step TB test
• Current state licensure to practice as an Occupational Therapist.
• Preferred working knowledge of HCHB
