Under the direction of the Director of Pharmacy, responsible for the full practice of pharmacy in accordance with applicable regulatory statutes and accreditation standards; interpreting prescription orders, packaging, labeling, dispensing, administering and distribution of drugs and devices; accepts full responsibility as Pharmacy Manager of the site as defined in Florida DPR Statute #465.018, and other requirements as defined in Statutes 465, 21s, 893, plus applicable DEA, AAAHC and 340b requirements.

CNA

– Case Management

The CNA – Case Management position is responsible for ensuring improved health outcomes through coordinating care by enhancing communication and the continuity of care. This position will consult with other health care team members to coordinate services for Community Health Center patients.

The CNA – Case management position will follow-up on assigned patients gaps in care, emergency room visits, hospital discharges, document in the Case Manager’s bucket and preventive care.

Pharmacy Technician

Under the direct supervision of the Pharmacist, the Pharmacy Technician is responsible for assisting the pharmacist in filling prescriptions, billing of prescriptions to third parties, collection of payments from patients and maintaining drug inventory.

Dental Assistant

Responsible for dental assisting duties listed in Florida Statutes 466 and Administrative Rules 64B-5, screening and preparing patients for treatment; assisting dentist in treating patients; educating patients; processing instruments; taking and developing radiographs.

Dental Hygienist

Responsible for delivering dental hygiene services in accordance with Florida Statute 466 and Administrative Rule 64B5. Delivers care at CHC sites and other off-site centers as needed, with an emphasis on prevention of and patient education about oral disease.

Optometry Manager

The Optometry Manager is responsible for direct oversight of all the Optometry sites to meet financial, quality, patient satisfaction goals, employee engagement and retention. Provide leadership, administration and coordination within the Optical sites to include clinical and non-clinical areas. Act as the liaison between the Optical sites business and the Regional Directors. Provide managerial leadership to ensure the implementation of the CHC Mission and Strategic initiatives.

Ensures optimum employee satisfaction by building and maintaining a working environment of trust, open communication and respect for each employee.

Dental Front Desk

Responsible for greeting patients, patient registration, financial counseling, referrals, establishing and maintaining accurate patient dental records in the computer. This position may require some medical and dental front desk responsibilities.

Vaccine Coordinator

Under the guidance of the Regional Nurse Manager the Vaccine Coordinator is responsible for the oversight and management of vaccines and Community Health Centers’ vaccine programs. The Vaccine Coordinator maintains vaccine integrity from storage to proper handling, training, and inventory process.

Women's Health Nurse Practitioner/Physician Assistant

The Women's Health Nurse Practitioner/Physician Assistant functions as a collaborative member of the health care team in the delivery of comprehensive health and health maintenance to patients in ambulatory settings. The Nurse Practitioner provides outpatient comprehensive primary care service including preventive and health maintenance services, acute and chronic care, clinical evaluation, diagnosis and treatment.

Responsible for preparing patients for interaction with provider, assists with coordination of quality patient care, lab work, administering medications and immunizations, and performing routine clinical duties as directed by the provider.

Nurse Practitioner

Provide outpatient comprehensive primary care service including: preventive and health maintenance services, acute and chronic care, clinical evaluation, diagnosis, and treatment as well as primary care consultation services.
